Complete all the first part of the form with the details of the person you are buying the membership for. These details will be used to set up their membership so please do not enter your details.

All the fields marked * are required for the membership to be created. You will not be able to submit the application if any of them have been left blank.

You will need to select their primary service. Don't worry, they will be able to change this and add additional ones later.

When you get to the question "How did you hear about Proficiency" select "Gift membership".

You will then be asked for your name so we can tell the new member who bought their membership and your email address so we can tell you when it is all set up. These are compulsory fields. You can if you wish also provide a message that you would like included in the welcome email but that is optional.

The "real person" check before you can submit the application is a sum. You just need to enter the answer (not copy the characters) and you will be taken to the payment portal.

Once your payment has been completed the portal will issue you with a payment confirmation. This is your receipt so please keep it safe.
